Glad to have helped. [url=http://www.allbusiness.com/forms/service/22/index.html?google=sample%20service%20agreements]AllBusiness[/ulr] might be one place to start with them. Most of the other sites I found seemed to want your email address before they allowed "free" access: Ultimate Service Agreement and Online Legal forms - just in case you have a "spam" email you can give them, no telling what you might be signing up for on some of them.

The All business one though has a preview which might give you some ideas though.

Yes, the first link was a great example.

On the topic, what about electronic signing of forms...
Is it as valid as a writen signature? Also do you require software for this or will a simple e-mail form be ok.

For example agreement is available through website, customer types name and/or other information than send's it to me through e-mail.

As far as electronic signing, I have not really figured that out. I have seen some companies say that by clicking here you agree to it. Some make you put your initials. And one company that I worked with - well they made you type your name in six times.

I wuld say usually electronic signing is valid - most people / companies are reocgnizing that as a legal way of business. Personallly, I would have it on my website and have them fill in a few blanks. I would not really rely on email at for that.

As for electronic signatures, the federal government mandated some years ago that e-signatures had to be accepted at some point and in some form, thought I don't know any specifics (such as a regulatory citation.) I am regulated by Customs & Border Protection, and they have mandated, probably as far back as the late nineties, that e-signatures were acceptable, be it a printed name or a scanned signature. That being said, the court of jurisdiction might take a different view.
